Avident Health LLC and Unite for HER have been awarded an independent quality improvement grant funded collaboratively by Pfizer Inc. and the National Comprehensive Cancer Network (NCCN). The grant will support a collaborative project deploying Avident Health's proprietary MyNavigator digital platform to enhance care delivery for patients diagnosed with metastatic breast cancer. The initiative aims to bridge critical communication gaps between clinic visits by delivering personalized, patient-friendly education aligned with NCCN Guidelines for Patients, driving active enrollment and utilization of Unite for HER's free supportive services, and equipping informal family caregivers to foster shared decision-making, treatment adherence, and enhanced quality of life.
Living with metastatic breast cancer requires complex, ongoing treatment management where adherence, symptom monitoring, and whole-person supportive care are paramount. However, patients and their loved ones often struggle to navigate complex clinical guidelines and access community-based supportive resources outside traditional clinical appointments. This grant-funded initiative addresses those gaps by turning the MyNavigator platform into an interactive, home-based companion.
The platform delivers tailored NCCN Guideline education by translating complex clinical pathways into accessible, bite-sized modules tailored to each patient's specific treatment journey, empowering patients to participate actively in informed clinical decision-making. It also streamlines access to integrative supportive care by directly integrating Unite for HER's comprehensive suite of free services—including professional nutrition counseling, fresh food delivery, mental health counseling, and evidence-based integrative therapies—into daily navigation prompts.
Uniquely, the platform engages the informal caregiver by including family members and daily care partners in a secure, proactive communication loop, ensuring they have the context, education, and early symptom alerting needed to support treatment adherence at home. Additionally, it enables proactive care team interventions by capturing patient-reported check-ins and symptom trends to alert clinical teams early, preventing avoidable complications and reducing acute care utilization.
"Navigating metastatic breast cancer is a daily reality that extends far beyond the four walls of the clinic," said Dr. Maen Farha, Avident Health LLC. "We are honored to receive this support from Pfizer and NCCN. By combining the proactive digital coordination of MyNavigator with Unite for HER's exceptional integrative services and NCCN's gold-standard guidelines, we are creating a continuous circle of support that empowers patients, supports clinical teams, and actively engages the family caregivers who are vital to treatment success."
"Unite for HER is dedicated to provide a supportive community and transformative integrative care directly to metastatic breast cancer patients," said Sue Weldon, Founder and CEO of Unite for HER. "Collaborating with Avident Health on the MyNavigator platform enables us to reach patients and their families directly at the exact moment they need guidance, removing barriers to our integrative nutrition, wellness, and emotional support resources."
The project will measure key quality improvement endpoints over the grant period, evaluating improvements in patient knowledge concordance with NCCN Guidelines, rates of supportive service utilization, patient-reported decisional conflict, and therapy adherence metrics. This initiative matters because it directly addresses persistent gaps in metastatic breast cancer care by integrating evidence-based education, supportive services, and caregiver engagement into a single digital platform. By aligning with NCCN Guidelines and leveraging Unite for HER's resources, the collaboration aims to improve treatment adherence and quality of life while reducing avoidable acute care utilization.
